Payment in the form of cash, check, PayPal, Venmo, or Zelle is due at the time of service, and you will receive a receipt containing all relevant insurance information to attach to your insurance form for submittal. I am an out‐of‐network provider and am not affiliated with any insurance plans or networks.
There is no extra charge for family or group sessions.
I do have a 24 hour cancellation policy and charge in the amount of a full session for missed appointments.
Sessions are conducted from my office; however, I do offer the convenience of virtual appointments.
All clients are required to sign intake forms before sessions can begin.
